I agree to allow Georgia Student Scholarship Organization (SSO), Inc (Georgia SSO) to submit my information as a substitute IT-QEE-TP1 form to apply for approval to the Qualified Education Expense Tax Credit Program to the Georgia Department of Revenue (GA DOR). Georgia SSO does not guarantee your request will be approved by GA DOR and any information rejected by GA DOR from the Form is the responsibility of the applicant. Georgia SSO does not warrant or guarantee the accuracy of the data provided by the applicant to Georgia SSO. If the information you provided does not result in DOR approval, verify your information from your tax return, create your Georgia SSO account and update your information to be resubmitted. To help fund your school’s scholarship program, I commit to fully funding my approved contribution amount within 60 days of DOR approval.
